Important Notice: Changes to Application Process
Effective May 14, 2012, applications must be sent in to Registry Connect by mail or courier only.
A completed and notarized Statutory Declaration for Proof of Identity must accompany every application, and must contain the signature, and the printed name or stamp, of the Notary or Commissioner of Oaths. Commissioners of Oath must include the expiry date of their commission.
For Alberta Birth, Marriage and Death certificates, contact Registry Connect. We are the authorized agent and service provider designated by the Province of Alberta, to assist all non-residents of Alberta in obtaining their Alberta Birth, Marriage and Death certificates or documents.
Registry Connect provides fast, efficient service. We offer 3 types of service options for you to choose from.
Gold Service Option (rush processing, courier delivery).
Cost - $39.64 per document fee, plus $25 rush processing service fee, plus the courier's delivery fee.
(Delivery cost varies according to the destination. Registry Connect will add the courier fee to the subtotal and charge your credit card accordingly).
Silver Service Option (priority processing, mail delivery).
Cost - $39.64 per document fee, plus $25 priority processing service fee.
Bronze Service Option (standard processing, mail delivery).
Cost - $39.64 per document fee only (no other fees will apply).

You may also require an Affidavit of Relationship to complete your application.
Print the Registry Connect application form and complete the appropriate sections.
Take your government-issued, photo ID to a Notary or Commissioner of Oaths to fill out the Statutory Declaration for Proof of Identity.
Please note Ensure that the Notary or Commissioner of Oaths who notarizes your Statutory Declaration includes their signature, printed name, stamp or seal (if they have one), and the expiry date of their commission.
Please note Any changes made to your Statutory Declaration (crossed or whited out information, or information added later in a different colour pen) must be initialled by the Notary or Commissioner of Oaths who notarized your form. Changes that have not been initialled, or have been initialled by someone other than the Notary or Commissioner of Oaths, will result in a returned Statutory Declaration.
Mail or courier the application to Registry Connect.
Have your certificate delivered to you by mail or courier (regular or rush service).
Registry Connect only provides service for people who live outside of Alberta and require Alberta vital statistics documents.
If you live within the Province of Alberta, you must apply for your document in person through an Alberta Registry Agent.
